Call * accepts self-referrals and referrals from friends and family, community agencies, and healthcare professionals

Eligibility / Target Population

People 16 years and older who are dianosed with an illness such as anxiety, bipolar disorder, depression, or schizophrenia, and are experiencing significant mental health challenges * must need help coordinating multiple services * must be willing to work toward recovery
