Allow paramedics to apply their training and skills in a community setting * person is identified as high risk for readmission * services may include:
home visits and monitoring wellness clinic influenza immunization clinics falls prevention cognitive screening ECGs independent living guide chronic disease management support and follow up medication review remote monitoring: blood pressure, glucose risk assessment trip and fall physical assessment from head to toe
