Requires initial assessment with Home and Community Care Nurse who visits community every Tue
Needs are then determined with Community Health Representative and Nurse

Eligibility / Target Population

Members of Seine River First Nation
Individuals who are considered elders and live alone or individuals who have had recent surgery or medical conditions which require assistance

Description of Services

Assists seniors to live independently in the community through the provision of nourishing meals at a reasonable cost

Coordinates with Community Health Representative and Cook to deliver homemade meals five days per week to individual's homes, suitable for diabetics, but may request meals for those on special diets

Meals are delivered by a volunteer who provides regular social contact and checks on the health and safety of the client
