Description of Services
Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff * monthly diabetes education groups offered virtually
The diabetes educator is responsible for assessing health needs, education and ongoing management for clients with type 2 diabetes only
- newly diagnosed and established diagnosis
- pre-diabetes
- insulin starts
- gestational diabetes and type 2 in pregnancy
- healthy eating, exercise and other lifestyle management strategies
- blood glucose monitoring
- risk reduction for complications of diabetes
- continuous glucose monitoring available
- diabetes specialist on site (physician referral required)
- free diabetes education and support groups
