Interdisciplinary team of doctors, nurse educators and coordinators, a social worker, a pharmacist, a dietitian and a chiropodist to promote the health of patients with advanced chronic kidney disease * provide education, advice and support related to patient's options for treatment and living with chronic kidney disease so patients can manage independently
