Hospital-based outpatient rehabilitation for people who have had a lung transplant * services provided by respirologists, anesthetists, thoracic surgeons, nurse practitioners, transplant coordinators, social worker, psychiatrist/psychiatric nurse, physiotherapist, and nutritionist

Progression and Maintenance levels of rehabilitative care for patients working to restore or maximize functionality, or prevent functional decline or injury, or to maintain functionality
