Description of Services
Community Referrals by EMS - All paramedics in Grey County provide direct referrals to the Ontario Health at Home South West Region if they believe a patient living in the community is at risk
Community Paramedic Home Visits - Community Paramedics visit referred patients from a local family health team who are diagnosed with chronic conditions such as diabetes, congestive heart failure and/or chronic obstructive pulmonary disease (COPD) in their home to assist them in managing their health
Community Paramedic Clinics - Community Paramedics hold health assessment clinics in designated Grey County public housing buildings as part of an ongoing study with McMaster University * blood pressure monitoring, health education and diabetes risk assessments are focused on in an attempt to reduce at-risk patients' dependence on the '9-1-1' system and local emergency departments
Community Palliative Support - Community Paramedics provide urgent palliative support to patients rostered with regional Palliative Care Program * community paramedics are trained in palliative symptom relief and can assist with IV and SQ line initiation
COVID Immunization and Testing Support - Community Paramedics are trained in PCR testing and vaccination supporting Grey Bruce Public Health Unit (GBPHU) * homebound COVID and influenza vaccinations are also available and coordinated through GBPHU
