Application
All referrals welcome.
Individuals referred are contacted within a two week period by the intake worker to schedule an intake and to come in to sign consent forms for release of information. Individuals must have a psychiatric diagnosis at intake. If there is a waiting list, individuals are contacted on a weekly to bi-weekly basis for a follow up and encouraged to utilize crisis intervention if in need.

Description of Services
Supports and services available through Case Management include:
- ongoing assessment and goal development;
- crisis intervention and outreach;
- supportive counseling and assistance with problem solving;
- assistance in accessing treatment and support in addressing symptoms and following through on treatment recommendations;
- coordination of services with natural supports and other agencies;
- assistance in meeting basic needs; and
- advocating with and on behalf of clients as necessary.
