Eligibility / Target Population
Client must be 55 years of age or older with a permanent physical disability or acquired brain injury * require assistance with personal care and activities of daily living * have the ability to direct own care * be a resident of Ontario with a valid OHIP * not be in receipt of insurance funds * qualify for Aging at Home programs

- dressing, undressing, bathing, washing
- rising and retiring
- meal preparation
- taking medication
- essential communication services
- for existing clients, palliative support can be provided when appropriate
