Description of Services

Provides services for individuals with complex health conditions in an outpatient setting. Rehabilitation programs include Musculoskeletal Rehabilitation, Neurological Rehabilitation, Total Joint replacement Rehabilitation, Seating clinic, Mindfulness based stress rection program and Pain management programs

  • Mindfulness Based Stress Reduction Program (Progression and Maintenance) 9-week program in which participants learn various meditation practices to help reduce pain and stress levels, increase activity levels, and improve self-esteem
  •  
  • Musculoskeletal Rehabilitation Program (Progression) Short-term intensive outpatient program for people with complex musculoskeletal rehabilitation needs * treats patients who have had orthopedic surgery for complex fracture, multiple trauma, rheumatic disease, or complex soft tissue injuries * program duration is approximately 6 weeks

  • Neurological Rehabilitation Program (Progression) Short-term intensive outpatient program for patients with complex neurological rehabilitation needs * treats patients with a recent acquired brain injury, stroke or neurovascular impairment or a neuromuscular disorder * program includes vocational rehabilitation, neuropsychology services, and a neurological peer support group for younger adults * program duration is approximately 8 weeks

  • Pain Management Program (Progression and Maintenance) 10-week chronic pain management program * uses cognitive-behavioural approach and physical activity to teach coping skills

  • Seating Clinic (Maintenance) Address both the basic and complex seating needs of patients who require the use of an assistive device to support mobility

  • Total Joint Replacement Outpatient Rehabilitation (Progression) 6-week hospital-based outpatient rehabilitation program for people who have had elective knee and hip replacement surgery * services include:

  • Young Neuro Survivor Peer Support Group (Progression and Maintenance) Support and education group for adults who have had a neurological diagnosis such as stroke, acquired brain injury, or neurodegenerative disease * facilitated by a social worker, with guest speakers attending on occasion * topics are guided by group interest and focus on areas of psycho-social adjustment after a neurological event * group meets biweekly for approximately 16 weeks
