Description of Services

Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services

  • Adult Day Program for Frail Seniors Adult day program * social, recreational, and therapeutic activities for frail elderly adults in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
  •  
  • Adult Day Program for Seniors with Alzheimer's Disease and/or Dementia Offers social, recreational, and therapeutic activities for Alzheimer's disease and other related dementias in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

  • Caregiver Support One-on-one and group support, as well as counseling for caregivers and families * family counselling * crisis intervention * support groups * training and education

  • Case Management Assessing, planning and coordinating services to meet a senior's needs, goals and expected outcomes * service may be provided at a older adults home or another location * advocacy to assist older adults or caregivers in accessing needed services

  • Friendly Connections Volunteer visits and/or phone calls to seniors and people with disabilities who live alone, to provide companionship and reassurance
    • volunteers promote companionship and support through one-on-one home visits and/or other regular phone calls to seniors in the community
    • program participants engage in social media activities to connect with family members and to broaden their network of friends and support systems
    • provided in several languages depending on volunteer resources available

  • Health Action Theatre by Seniors Social, recreational, and educational activities * can include crafts, cards, educational lectures, and outings * promotes positive problem-solving * culturally sensitive

  • Health Promotion and Education Program designed to provide education and health related activities/services that promote wellness for older adults and the aging process to caregivers, students and volunteers

  • Home at Last Program (HAL) Help older adults leaving hospitals to arrive home * offer clients additional support for up to 12 weeks after transitioning home from hospital
  •  
  • Home Help or Homemaking Assistance with daily chores such as laundry, light housekeeping, errands, essential grocery shopping, meal preparation and personal care * service is offered weekly, bi-weekly, monthly or seasonally
  •  
  • Meals on Wheels Hot meals delivered around 12 noon Mon-Sat or chilled meals delivered on Wed and Fri * special diet types available. Chinese and South Asian meals available.

  • Parkdale Assisted Living (PAL) A service that provides on-site support services to help older adults and people with disabilities with their daily activities

  • Respite Care Temporary relief for caregivers of people with complex medical conditions

  • Seniors Active Living Centre Community centre for older adults

  • Transportation Non-emergency transportation to local and long-distance medical appointments and regular activities such as grocery shopping and banking * volunteer drivers
