Description of Services

Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health

Primary Health Care Services -
Health care team includes doctors, nurse practitioners, registered nurses, dietitian, physiotherapist, kinesiologist, social workers, chiropodist (foot care specialist), health promoter and community developer * in-home palliative care services

Chronic Disease Self-Management - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions (open to everyone)

Smoking Cessation - One-on-one support to help quit smoking (open to everyone)

Free Seasonal Flu Vaccines - Available for all community members * drop in and bring Ontario Health Card

GB Works Needle Syringe Program - needles and other injection drug supplies provided and accepted for disposal * offered in partnership with Grey Bruce Health Unit

Free Tax Preparation Clinic - Offered through the Community Volunteer Income Tax Program of Revenue Canada * locations in Chatsworth, Dundalk, Durham, Flesherton, Holstein, Markdale

  • Foot Care Clinic Chiropodist (foot care specialist) and nurses assess and treat foot problems * service is provided to people who are at a higher risk of foot complications, such as people with diabetes, peripheral vascular disease, and immune-compromised

  • Medical Equipment Service Provides free used medical equipment and mobility aids in good condition such as walkers, canes, commodes, crutches * equipment can be kept or donated back * accepts new and used equipment from the community * waiting list kept for equipment requests

  • Nutrition Counselling Nutritional information and education * one-on-one, couple, or family sessions for people with food and health-related concerns
