Description of Services

Companionship, respite care, and emotional support to people and their family/caregivers dealing with a life-limiting illness * bereavement support for anyone who has experienced the death of someone close 

Bereavement Services -  Offers emotional, physical, spiritual and practical  support to each person living with illness and their family. Purpose is to support each family member through their time of illness, death and bereavement according to their needs and wishes. 

Hospice programs offer both group settings as well as one-to-one support, both in-person and virtual. All Hospice programs are offered by self-referral and are free of charge.
 
Services include:
  • health system navigation
  • caregiver support
  • volunteer companionship
  • support groups
  • grief and bereavement care for both children and adults

Palliative Care Community Team (PCCT) -  PCCT offers guidance through the complex healthcare system, offering expert palliative care consultation to members of the care team, providing access to palliative care professionals round the clock, ensuring continuous medical support as well as providing emotional support to families navigating the challenges of living with illness.
