Description of Services
Assists people living on low incomes, suffering from chronic health conditions, facing mental health issues, isolation and/or language barriers
Services include:
Services include:
- development of individualized care plans addressing health, housing, safety needs and referrals to appropriate service providers
- assistance completing applications for Canada Pension Plan, Ontario Disability Support Program, Ontario Works, passports, other identification
- supportive counseling
- advocating with case workers, housing and government agencies for effective solutions
