Description of Services
Supports for people living with prediabetes or diabetes
- Offers education for prevention and self-management support to fit an individual's lifestyle
- Provides access to a registered nurse and registered dietitian
- Provides referrals to other health care professionals
- Provides supplies to manage diabetes
- Blood sugar, HgA1c, blood pressure and oxygen monitoring
- Client support groups
- Community screening
- Foot care workshops/clinics
- Health fairs
- Presentations to the public and to health care providers
- School programs and visits
- Provides access to specialized medical care using video conferencing
- Allows specialists to remotely examine and prescribe treatments to patients in their home communities
- Reduces the need to travel to receive medical care
