Description of Services
Supports for people living with prediabetes or diabetes * one-one-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse, dietitian, and clerical support on staff
Diabetes education teaches patients how to become their best advocate in the management of their own health by providing resources that they need to maintain the healthiest life possible.
