Indigenous individuals and families of any age living with diabetes of every type and at every stage or at-risk within Dilico jurisdiction, and children in Dilico care and their caregivers

  • Provides an in-home and community-based diabetes and chronic disease management program
  • Services include:
    • Advanced and basic foot care
    • Diabetes care plans
    • Diabetes self-management
    • Nutrition and lifestyle education
    • Self-monitoring of blood glucose
