Description of Services
Provides education and services for the prevention, management and treatment of diabetes
- Provides case management and screening services
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and management of diabetes
- Assists individuals in finding appropriate services
- Provides referrals to other health care professionals
- Annual poker walk for all ages with grand prizes
- Basic foot care clinics
- Breakfast and lunch programs
- Community kitchen
- Fitness and nutrition programs
- Glucose monitoring
- Pre-natal education
- School diabetes program
- Support groups
