Description of Services

Coordinates care, linking patient and family members through early referrals to community services for non-medical management from the point of diagnosis, throughout the continuum of the disease

Organizes and coordinates dementia education training sessions

Provides active follow-up and monitoring of patients and their family caregivers to assist in reducing the number of patient crises

Works to reduce caregiver burnout and premature long term care placement

Maintains statistics and evaluates patient and family caregiver outcomes

Primary care practitioners
