Description of Services
Provides community-based, culturally appropriate services to urban Indigenous elderly, physically disabled or chronically ill individuals to promote independent living and an improved quality of life
Indigenous Support
Provides clients with:
Arranges visits from support worker in individual's home or long term care facility
Security Checks
Provides isolated individuals with regular contact (by telephone or in-person visit) to determine that the client is not in crisis
Indigenous Support
Provides clients with:
- information and referral services
- advocacy translations
- assistance with filling out forms and applications
- assistance with accessing services from mainstream organizations
- Provides transportation on an as needed basis to events at the Centre, appointments at the hospital or clinic and for errands around town including grocery shopping
- Organizes a twice-monthly luncheon
- Provides story telling, activities and refreshments for program participants
- Provides transportation for attendees
