- 17 years of age and younger
- a resident of Ontario and
- a member of a household with an Adjusted Family Net Income (AFNI) at or under the levels shown here

- Required to present status card
Ontario Disability Support (ODSP)
- Required to present dental card issued by ODSP
Ontario Works (OW)
- Requires dental card issued by the Ontario Works' office; proof of eligibility for the month must be shown

Description of Services
Provides basic preventive and restorative dental services through a mobile dental office for children and adults who do not have access to dental care
- Provides dental services for recipients of the Healthy Smiles Ontario Program (HSO), Ontario Disability Support Pension (ODSP), Ontario Works (OW) and Non-Insured Health Benefits Program (NIHB)
- Provides information on preventive dental health practices
