Referral can be made by individual, community person or professional though The Access Point Northwest
Eligibility / Target Population
- Individuals 18 years of age and older with severe and persistent mental illness who need continued assistance and support to live in the community
- Individuals 18 years of age who are dependent on one or more substances (alcohol, prescription drugs, cocaine, opiates, cannabis) and suffer from mental illness (schizophrenia, mood disorders) will be considered

Description of Services
Utilizes an outreach, community-based case management approach to help clients with severe and persistent mental illness and clients with mental illness and co-occurring substance use disorder overcome problems related to problematic alcohol/drug use or dependence and stabilize their mental illness
