Eligibility / Target Population

Must have a valid Ontario Health Card
Must have health care needs that cannot be met with any combination of caregiving, care in your home or care in your community
Must have health care needs that can be met in a long term care home

Fees

Rates set by the Ontario Ministry of Long-Term Care * subsidies for basic rates may be available for eligible applicants * some programs/services may require additional fees

Description of Services

Operates a 9-bed long term care facility offering a variety of programs, services and amenities
Coordinates and delivers the following programs to residents:

Community Special Events
Facility Tours
Guest Speakers
Intergenerational Programs
Memorial Services
Pet Visitation/Therapy
Volunteer Appreciation Events
Voting Day Polling Station

Provides residents with the following services:

24-hour Nursing Care and Supervision
24-hour On Call Physician
Care Conferences
Foot Care
Hairdressing/Barber Services
Laundry Services
Meals
Medication administration
Palliative care (end-of-life care)
Physician Visits
Social Work Services/Family Services Coordinator
Spiritual Programs and Pastoral Care
Various recreational programs and social events
