Provides services and care to individuals who have physical disabilities, serious health issues, or those who are frail and/or elderly
Assists individuals with the following:

- Access to cultural activities, Elders and Healers, traditional foods and ceremonies
- Advocacy with community agencies
- Assistance with housing issues
- Home visits/friendly visits
- Medical transportation and accompaniment services
- Preparing forms and applications
- Referrals to community services and Friendship Centre programs
- Security checks
