Application

Physician referral required for clients admitted from acute care and/or the community
If an individual doesn't have a family physician, referrals can be made by a walk-in clinic doctor, community health clinic, the local hospital, or Ontario Health atHome - North West Area

Eligibility / Target Population

Seniors 65 years of age and older who are experiencing complex health problems, including

Multiple admissions to the hospital or multiple visits to the emergency department
Cognitive changes
Changes in health status or functional decline

Note: Individuals must be medically stable to participate in the program and have the cognitive capacity as well as potential stamina and endurance to participate in daily therapies

Description of Services

Provides a low to medium intensity rehabilitation program to help older adults improve their abilities to live in their own homes with or without support

Focuses on the clients medical, functional and cognitive needs in order to optimize their well-being

Refers individuals to additional supports, such as a Psychologist, Psychiatrist, Registered Dietitian, Speech Language Pathologist, or Spiritual Care Associate if needed

Works with Ontario Health atHome - North West Area to facilitate discharge back to the community with formal support services
