Individuals 18 years of age and older with a primary diagnosis of a serious mental illness or concurrent disorder
Must be in need of case management services
Resident of Thunder Bay
Must qualify for Rent Geared to Income Housing

Description of Services

Provides housing for individuals with a serious mental illness or concurrent disorder (mental illness and addiction)

Provides subsidized housing and case management services

Ensures the individual is living responsibly by providing regular home visits from a case manager

Note: Does not provide emergency or crisis housing
