Provides the following services:
- Home visiting nurse program
- Companion, homemaking, and personal care
- Exercise and Falls Prevention Classes
- Social Work services in the community setting
- Operates 4 days/wk
- Provides a therapeutic program, day respite and socialization
- Provides transportation if required
- Offers Corporate Wellness programs and Private Pay
