Eligibility / Target Population
- Adult services for individuals 18 years of age and older
- Pediatric services for children and youth 17 years of age and younger
Services and programs for any of the following:
- Persons with Type 1/Type 2 Diabetes
- Gestational Diabetes
- Type 1/Type 2 Pregnancy
- Impaired Glucose Tolerance
- Impaired Fasting Glucose
- High Risk persons

Description of Services
Offers a diabetes management and prevention program to assist individuals in understanding and managing their illness and to learn prevention measures
- Provides both adult and pediatric services
- Offers individual and group educational sessions and health promotion activities
- Accepts walk-in patients
- Access to Traditional programs and medical interpreters
- Basic and advanced foot care/assessment
- Client support groups
- Community screening
- Cooking classes
- Exercise programs
- Grocery tours
- Medication counselling and insulin teaching
- Presentations to the public and to health care providers
- School programs and visits
- Dietitian (Adult and Pediatric)
- Exercise therapist
- Foot care provider
- Nurse
- Wellness counsellor
