- Provides access to other health care professionals within and outside of the Family Health Team
- Coordinates documentation if patient receives care from a member of the team
- Chronic Obstructive Pulmonary Disease (COPD) or Asthma Program

About Us
An inter professional group of health care practitioners, including family physicians, assisted by locums (short-term physicians that visit our community) provide health care in an integrated, respectful and holistic environment to help keep patients healthy before illness occurs. The focus of the family health team is on disease management and prevention, rehabilitation, palliative care and health promotion.
