Description of Services
Offers a new way of monitoring and supporting individuals with congestive heart failure (CHF) and/or chronic obstructive pulmonary disease (COPD) from their homes
- provides individuals with equipment/kits necessary to measure vital signs and health information
- equipment/kits are delivered and set up at the individual's home by a technician who also provides training
- individuals submit daily health status updates
- nurses monitor the data and take action if the numbers or the individual's responses to a list of questions indicate a need for intervention
- provides health coaching sessions with all individuals on a weekly basis
