Description of Services
Diabetes education and management support by Dieticians, Nurses, Endocrinologists and a Social worker for people living with prediabetes or diabetes including:
- gestational diabetes
- pre-existing diabetes with pregnancy
- post-gestational diabetes
- type 1
- type 2
- insulin pump therapy
- blood sugar monitoring monitoring (including Libre and Dexcom)
- inpatients with diabetes
