People who need end-of-life care or bereavement supports * must live within geographic service of hospice (Steeles Avenue to the Lakeshore, Yonge Street to Victoria Park) * 18 years of age or older

Description of Services
Supports clients and their caregivers at the end stage of life * practical, physical and emotional support to maintain comfort and quality of life for the client as a compliment to professional medical care
Services include:
Bereavement support for anyone who has experienced the death of someone close * services can be provided at home, or in hospitals and long-term care homes
Bereavement Services - Support groups held in various communities throughout the year
Services include:
- case management
- visiting volunteers
- companionship
- respite care
- emotional support to people and their family/caregivers dealing with a life-threatening illness
