Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring:
Outreach Clinic at Bruce Power
Program is delivered by Grey Bruce Health Services
- blood sugar levels
- medications
- healthy eating
- exercise
- other lifestyle choices
- registered nurse
- registered dietitian
- nurse practitioner (all certified diabetes educators)
