Description of Services

Supports for older adults, and adults with disabilities * helps people remain independent in their own homes and community for as long as possible
  •  
  • Alzheimer and Dementia Visiting Regular visits to people with Alzheimer's disease or dementia, for respite and companionship by trained volunteers
  •  
  • Caregiver Training, Information and Education Peer support groups, education, and training for family members, service providers, and caregivers * sessions may include information such as advanced directives, safety issues, housing options, caregiver wellness and more
  •  
  • Congregate Dining Monthly social activities * some events include a hot meal * activities can include games, crafts, and physical activities
  •  
  • Hospice and Bereavement Services Companionship, respite care, and emotional support to people and their family/caregivers living with a life-threatening illness at any stage of illness from diagnosis on

  • Hospice Volunteer Visiting Trained volunteers provide support, respite and emotional support to people and their family/caregivers living with a life-threatening illness at any stage of illness

  • Security Checks Regular visits and phone calls to check on the health and safety of people who live alone * screened volunteers listen, talk, and help isolated people stay in contact with the outside world

  • SMART Falls Prevention and Group Exercise Classes for Seniors Group exercise and falls prevention classes for older adults and people with mobility issues and chronic conditions * aims to improve strength and balance to prevent injury and falls

  • Volunteer Visiting Regular volunteer visits to older adults and people with disabilities, who live alone * volunteers visitors are matched one-to-one with clients to provide companionship and support * visits may also include activities outside the home
