Description of Services
Supports for individuals and their families, who have been diagnosed with cancer and other life-limiting illnesses * referral to other community services
Services are provided at home, hospital, hospice and long term care homes
Education and consultation to service providers offering palliative and end-of-life care in home, hospital, hospice suites or long term care homes
- provides supportive care based on clients and family needs through supportive care coordination and trained volunteers
- practical psychological, emotional, social, spiritual and bereavement
