Eligibility / Target Population

Needs to be willing to develop and work toward a rehabilitation plan * needs to be able to care for own physical needs, climb stairs, take medication independently, and maintain basic daily living skills * not be considered a threat to others

Description of Services

3 24-hour moderate support homes with a minimum of 8 hours of support per day
  • Youth home - co-ed: Ages 16-24 * 5 beds * staffed
  • Female-only home: four beds
  • 4 co-ed homes: permanent * low-support * 12 beds

Shared living environment * goal of reintegrating clients into community living * focuses on recovery for persons who live with a mental illness * housing provides an environment typical of community settings and expectations

Transitional Youth Apartments - recovery-focused program * reintegrates youth back into community living * bachelor or one-bedroom units * staff on site for support
