Treatment, rehabilitation and support through an intensive service coordination model to persons living with severe and persistent mental illnesses * services include outreach, injections and psychiatry * facilitates a support group for family and loved ones of individuals living with mental illness * education, coordination, consultation and support regarding Community Treatment Orders provided
