This team offers assistance to patients and their families who are living at home with a serious illness. The team is made up of Palliative Care physicians, Advanced Practice Nurses, Occupational Therapists, Spiritual Counsellors, Social Workers, Pharmacists, Physical Therapists, Dietitians, and Chaplains who work closely to ensure patients receive the best possible care at every stage of their disease.
