Although initial inquiry may come from patient/family member, the formal referral must come through a hospital professional such as a physician, social worker or nurse.
Waiting Period - Case management services are available prior to discharge from hospital
Admissibilité / population desservie
Must be a resident of Ontario with a valid Ontario Health Insurance Plan (OHIP) card. Must be a patient in the Oshawa Hospital. Further eligibility will be assessed upon initial intake according to the Long Term Care Act 1994 (see attached).

Ontario Health atHome provides in-home and information and referral services by care coordinators in the Lakeridge Health Corporation - Oshawa Hospital.
The role of the hospital care coordinator is to assess and arrange for required Ontario Health atHome in-home services upon discharge from hospital and to link patients to other useful community resources.
In-home services may include personal care, nursing, physiotherapy, occupational therapy, speech language therapy, dietetic services and social work services.
