Call in to the office to special to a School Health Support Services Care Coordinator to make a referral. Valid Ontario Health Insurance (OHIP) number is required. Request for Assessment Form, Speech Pathology Referral Check List, School Check List

Services are available province-wide to children 21 years of age or younger with special needs in a school registered with the Ministry of Education and whom are Ontario residents insured under the Ontario Health Insurance Plan (OHIP).
