Participants must be 18 years of age or older and have suffered one of the following events in the past year: heart attack, by-pass surgery, cardiac transplant, angioplasty, stable CAD/angina, valve replacement, congestive heart failure, cardiomyopathy

Services are provided without direct costs upon presentation of a valid OHIP card
