Supports patients who need additional behavioural and caregiver support to return home safely * along with rehab and short-term care, the program includes behavioural strategies and caregiver coaching to help manage challenges at home * goal is a smooth, well-coordinated transition that promotes safety, confidence, and recovery in the comfort of home * 26 week program
Designed for patients who are medically ready for discharge home from hospital, but require additional behavioural and caregiver support beyond standard home care * offer caregiving (personal support worker) as well as caregiver support/relief as required (social worker)
