Live in the area served * be a member of one or more of the following priority populations: Youth (ages 13-25) * adults 65 years and older * newcomers to Canada (less than 5 years) * racialized communities * people facing mental health conditions or substance use issues * people who are homeless or at-risk of homelessness * families who are under-resourced Mobile services: 65 years of age and older * having difficulty accessing primary health care due to physical, cognitive or psychiatric difficulties * willing to transfer their primary health care to Bramalea Community Health Centre * not living in a nursing home facility * not requiring palliative care * ability to provide consent to participate and undergo an intake assessment

Primary health care focusing on health promotion and prevention to the residents of Bramalea * services include disease prevention, management and treatment, as well as rehabilitation and health promotion for all stages of life * primary healthcare teams offer:
one-on-one appointments for individual clients
shared medical appointments with a family doctor or nurse practitioner and a group of clients who have a similar condition
group education sessions - focus on specific topics and goals for clients and community members (pre-registration is usually required)
pregnancy care for clients who are up to 32 weeks pregnant
immunization: Immunizations for adults and children under the Publicly Funded Immunization Schedule for Ontario
cancer screening is available based on age and other risk factors, for colon cancer, breast cancer, cervical cancer, etc
Interdisciplinary team of health professionals: physicians, nurse practitioners, support and care navigators, dietitian, diabetes nurse educator, physiotherapist, health promoter, medical office assistants, social worker/counsellor collaborate to provide an approach to health care to address health and social needs
