Release planning case management to non-violent adults with a serious mental illness or serving sentences in the Stratford Jail, Elgin Middlesex Detention Centre, St Thomas Forensic Unit and Central North Correctional Centre

works with incarcerated individuals and their support person(s) to formulate a plan intended to increase the success of community reintegration
connection to resources and psychiatric services
urgent consultation to families, individuals, service providers, particularly police officers, when dealing with an individual who has, or maybe about to commit an offense
