Provides individualized support to clients living with a co-occurring developmental disability and a diagnosis of serious mental illness. The team also provides dedicated support to individuals with a dual diagnosis who also have justice system involvement. The supports offered to clients typically include: comprehensive assessment, collaborative development of a personalized safety plan, collaborative development and implementation of an individualized recovery/goal plan and assistance to achieve/maintain optimum health and wellness.
