Offers direct assistance for adults with a developmental disability who are living on their own

Assists adults with an intellectual disability to acquire skills for daily living and to access any community supports and services

Provides the following services:

Case management
Assistance to coordinate community resources
Development of service plans with other service providers
Emotional support during stressful life changes
Information about other available resources, supports and services
