Provides an interdisciplinary assessment consisting of physician, physiotherapist and occupational therapist for individuals with post-COVID condition * involvement of other services such as social worker and speech language pathology will be determined by the assessment team * treatment is focused on helping patients develop tools to manage their symptoms, return to activity, and improve their quality of life
