Referrals for the Health Links program come from the West and South Health Links teams and the ACT Team at Reconnect, and COTA’s Toronto West Integrated Service Team (Case Management). Referrals are processed through The Access Point.

Offers subsidized independent units with visiting residential support to individuals with serious mental health issues. Tenants have access to residential support and case management support. Tenants supported by Reconnect Community Health Services also have access to clinical support.
