Supports for people diagnosed with a life-limiting illness and those coping with the loss of a loved one * services may include:
- consultation with hospice palliative physician in partnership with family doctor to discuss pain and symptom issues
- individual needs assessment for supportive, spiritual and advanced care, as well as end-of-life planning
- one-on-one consultation with supportive care coordinator to meet spiritual and psycho-social needs
- grief and bereavement supports for people with a life-limiting illness and those dealing with the loss of a love one
