Admissibilité / population desservie

Patients 18 years of age and older of the Family Health Team who meet three or more of the criteria for Metabolic Syndrome including the following:

High blood sugars
Abnormal cholesterol
High blood pressure
High triglycerides
High abdominal body fat

Lifestyle intervention program

Offers a six-month program offered by a team of healthcare professionals including a Dietitian and Physiotherapist
Offered in a group setting of 15 participants
